Machine Readable Files

The US Departments of Treasury, Labor, and Health and Human Services (the Departments) released the Transparency in Coverage (TIC) Final Rules public disclosure requirement, which requires Recology’s group health plan to disclose in-network provider negotiated rates, historical out-of-network allowed amounts for providers, and in-network negotiated rates and historical net prices for all covered prescription drugs at the pharmacy-location level (Note the requirement related to prescription drugs is deferred indefinitely). These disclosures must be made through three machine-readable files posted on an internet website in a standardized format, updated monthly.

Notice of Availability of IRS Form 1095‑C

In accordance with federal law, Recology, Inc. is required to provide IRS Form 1095‑C to certain current and former employees. This form contains important information about the health coverage offered to you and your family. If you were a full‑time employee for any month in 2025 or were enrolled in our self‑insured health plan, you may be eligible to receive this form.

You may request a copy of your Form 1095‑C at any time.

To request your form:

Please include your full name, current mailing address, and the tax year for which you are requesting the form.
